How to Submit a Referral (order/request) to IHCS

Welcome to IHCS. We recognize the vital role providers play in ensuring patients receive timely, high-quality care. This guide is designed to make the referral process straightforward, consistent, and efficient across all available channels.

Please send all discharge orders, prescriptions and/or medical orders directly to IHCS using any of the methods below:

Option 1: CarePort users

You can submit referrals via the CarePort platform for Home Health, DME and IV services.

Follow your normal process and look for Integrated Home Care Services (IHCS) when identifying the provider to send the orders to.

Option 2: Parachute Health

(Ohio and South Carolina markets only)

You can submit DME: Referrals to IHCS. Follow your normal process to log into Parachute Health.  Once you enter all the patient information, a dropdown of suppliers contracted with IHCS will appear and you can choose the supplier of your choice.

Option 3: Fax

Submit complete discharge orders/prescriptions and/or medical orders via fax at 844-215-4265 for Home Health, DME and IV services.  If you need additional information on what constitutes a complete order, see required information below, or contact IHCS at 844-215-4264.

Option 4: Phone

Use for urgent or real-time support.

Instructions:

  1. Call referral line: (844) 215-4264.
  2. Information to have ready: Patient demographics, diagnosis, and clinical details.

Required Information for all Medical Orders

Patient information:

  • Full name, DOB, gender
  • Health plan and ID number
  • Complete address and phone number
  • Diagnosis and ICD-10 code

Ordering provider information:

  • Full name and group name
  • NPI number
  • Contact information

Clinical documentation:

  • Signed physician’s orders
  • Face sheet (if recently discharged)
  • History and Physical

Authorization details:

  • Specific services required

 

Post-Referral Process

  • What happens next: Referrals are reviewed by IHCS
  • Follow-up communication: IHCS will notify referring providers during and after authorization.
